What is the process for referring a person with learning difficulties and complex needs into CHOICE homes?
Referrals come from a wide variety of backgrounds including placement breakdowns, SEN schools, residential colleges, community support units and individuals looking to leave the family home for the first time.

Each person referred into CHOICE is an individual whose needs, abilities and personality are assessed in detail in order to ascertain their suitability for placement within a CHOICE home. Referrals can be made by a Social Worker/Care Manager or directly from parents themselves. However, before assessment can take place the Care Manager acting on behalf of the service user must be consulted.

In addition to the assessment carried out by CHOICE, the referral process also includes inviting family, friends and Care Manager to visit any suitable homes to see for themselves the environment and services available. Where it is not possible or appropriate for the service user to visit in person, it is important to be guided by someone who knows the individual well and can advocate on their behalf.

Should all parties, including importantly, where at all possible, the service user themselves, agree that the placement is appropriate, the care manager will go to panel for funding. If, and when, funding is agreed a transition plan can then be devised.

A service user’s transition plan is specifically tailored to their individual needs and provides as much support as is possible to aid the person in the move to their new home.

This thorough referral process undertaken by CHOICE allows the service user, and all those supporting them, to feel as confident as possible that the placement will be a success.

Upon moving into their new home, the service user will receive a holistic approach to their needs and will be afforded a high standard of care and support to allow them to lead a fulfilling and active lifestyle.
