Service User Plans

We are pleased to report that we are in the process of implementing our Service User Plans to comply with National Care Standard 6.
The new package contains a fully detailed working care document of all support required to meet the individuals needs. In addition to this there is a summary version identifying all current action plans for the individual including Skills Development, Risk Management Summary, Health Action Plan, “All About Me” and consent forms.
The new Service User Plans are currently being piloted at Stoke Lodge, with plans to implement them across the wider group during the early part of 2005.
We have also reviewed our Risk Assessment system in order to bring it up-to-date. The Psychology team are currently compiling Risk Management Plans for each service user, which will be in place by the end of January 2005. The new format brings together information normally found in different places and presents it in a more user-friendly form.
